By submitting this form, I understand that participation in Service Club events poses the potential for injuries and damages. In any event of personal physical injury or illness, at my request (if able to make such a request), I release and hold harmless the organizers (St. James Service Club, Inc.) and other volunteer participants in getting medical assistance. I will be fully responsible for such expenses associated with any such medical attention, if warranted. I hereby volunteer for this activity and to its intent and purposes. To the fullest extent allowed by North Carolina law, I hereby waive all rights, discharge all claims, and release from any and all liability the organizers and all other participants, volunteers or otherwise in any St. James Service Club event.
